The unexpected arrival of Irene Adler (Whoopie Van Raam) and her son Angus (Sebastian Billingsley-Rodriguez) who may, or may not, have been fathered by Sherlock Holmes introduces a case with the staff splitting up to diagnosis the patient’s condition and also prove or disprove Adler’s paternity claims. Although initially believing Irene’s story, and see Holmes in her son, Watson (Morris Chestnut) begins to see the cracks realizing all of this is nothing more than one more scam in which Irene has involved an adept student.

This time, however, despite the hallucinations, Watson is clear enough to suspect the medication and the most likely source of its tampering. Putting his career on the line, while attempting to hide what he’s doing from the team, Watson performs an unapproved medical procedure which saves his patient’s life leaving the team, and his ex-wife, to face the ramifications.

Although we get threads involving Watson (Morris Chestnut) and Shinwell (Ritchie Coster) doing more odd jobs for Moriarty (Randall Park), the less effective aspects of the show are mostly kept on the backburner in “Patient Question Mark” in which Watson’s team searches out the family of Sasha’s (Inga Schlingmann) medical school cadaver to screen them for possible cancer. The journey will lead the group to Amish country, save the life of a young boy, and reunite a single mother with a larger family. Although not shown much on-screen, the trip also brings back memories for Watson and better days in his relationship with Mary (Rochelle Aytes).
